Reservation Form

The package includes:

• 10 nights accommodation at Atlantis Dumaguete.
• Airport transfers to the resort from Dumaguete. The traveler is responsible for arranging transportation to Dumaguete, Philippines.
• All meals at Atlantis Restaurant (chef-prepared breakfast and three-course lunch & dinner).
• Coffee, tea and filtered water are available from 06:00 am to 09:30 pm (Italian and speciality coffees are not included after breakfast).
• Unlimited internet access.
• Unlimited scheduled boat diving (up to four-day dives and one night dive per day); excludes dawn dives and additional day trips.
•  One (1) day trip to Apo Island or similar.

The package does NOT include:

• AIRFARE. The traveler is responsible for arranging transportation to Dumaguete, Philippines.
• Gratuities.
• Government taxes and fees.
• AIRFARE. I know we listed it twice. It’s that important.
• Everything else in the world that’s not explicitly included above.
• Optional Oslob Whale Sharks Trip US$131.00 per person.
• Nitrox US$219.00 per person
• Marine Park Fees to be billed at checkout (fees were $3.75 per day dive and $9 per night dive at the time of this writing, but may change)
• Rental Equipment.
• Land Excursions.
• Dive or travel Insurance.

Payments and Deadlines

• A $300 per-person deposit is required to complete your reservation and guarantee your spot.
• All deposits are non-refundable.
• November 17, 2025  is the last day to cancel your reservation without penalty. After that date, cancellation may result in forfeiture of a portion or all of the package cost. In case of emergency, a new guest can be substituted on the reservation without charge.
• First payment is due October 16, 2025. This payment is 50% of the total reservation cost. After that date, if we have not received your payment, your reservation may be cancelled.
• Full balance is due February 16, 2026.

Agreement to Terms and Conditions

Acceptance of all package terms and conditions and the SeaHorse Dive Club Release and Indemnity Agreement is required to participate in this excursion. This acceptance covers all guests indicated on this form.

Note: SeaHorse club members and previous travelers on SeaHorse club trips have an exclusive sign-up period of one month, until July 22, 2025. If you do not qualify, submit your reservation anyway. Your reservation will be confirmed once the period ends.
